Our service delivery is guided by a carefully designed holistic care plan developed by a Registered Nurse, that also incorporates each person’s individual goals and strategies for achieving them. We set emphasis on the importance of each person and their family being directly involved in choosing how the care is delivered, whilst being guided by a multidisciplinary support team.
